We require your name, date of birth, address, medical number and mobile number and details of whether you pay for your treatment under the NHS or whether you are exempt from paying (please provide details of the specific benefit that entitles you to free dental care).
Once we receive your details and have inputted them onto our system we will send you a medical history form and covid screening form to complete.
